How to Lodge an Injury Insurance Claim

If you are injured during an Over 45s League match, please follow the steps below to ensure your injury is recorded correctly and your insurance claim can be processed without delay.

Important Notes

  • Only injuries sustained during  official Over 45s matches are covered

  • Claims are subject to insurer terms, conditions, and excesses

  • Claims will only be processed by the insurer when you have incurred and provided receipts for out-of-pocket medical and some non-medical expenses

  • Reporting injuries promptly helps protect your eligibility

  • Training, unsanctioned practice matches and any out-of-season activities are not covered by this insurance policy.

Step 1: Report the Injury on Match Day

(Very Important)

All injuries must be reported to the referee at the time of the incident or immediately after the match.

The referee will submit an official Referee Injury Incident Report, which is required for insurance and competition records.

⚠️ Injuries that are not reported to the referee may not be eligible for insurance cover.

Step 2: Complete the Player Injury Notification Form

(Required)

As soon as possible after the match, the injured player must complete the Player Injury Notification Form

Forms received after 45 days from the incident may not be eligible for insurance claims.

This form:

  • Notifies the competition of your injury

  • Links your injury to the referee’s report

  • Allows the organiser to assist with insurance and welfare follow-up and sign-off for insurance claim to be lodged.

Step 3: Seek Medical Treatment

If medical treatment is required:

  • Attend medical provider of your choice

  • Keep all receipts, invoices, and medical reports

  • Obtain written confirmation of diagnosis and treatment where possible

These documents will be required for your insurance claim.

Step 4: Insurance Claim Process

Once your injury has been approved for a claim to be lodged:

  • The competition organiser will provide instructions on lodging an online insurance claim

  • You will be required to provide supporting medical documentation including out-of-pocket medical receipts.

Incomplete or late submissions may delay assessment.

Step 5: Claim Assessment & Follow-Up

  • Claims are reviewed directly by the insurer

  • Additional information may be requested

  • Assessment timeframes vary based on injury type and documentation
